This is the Letter of Authorization your current carrier needs. Everything on it has to match your existing bill exactly, which is the single most common reason a port is rejected.

Account Number *From your most recent bill. Port Out Pin *Also called a transfer PIN or passcode. Your current carrier can give you this.
